This sucks. I workout at hotel gyms often. Treadmills (from which he fell) can be treacherous. Especially dangerous when you're not accustomed to the equipment - as they vary. Sometimes they're not well maintained either.

Added: This guy was in his 40s. The most common reason for falling from treadmills is placing one foot down a little off to one side where it doesn't move along with the belt. This usually results in a nasty fall backwards as one looses balance while the belt continues moving. I've done it a couple times while running. Better warning signs (like "Dont use cellphones") would help along with better designs. Most newer ones have a safety clip you attach to yourself that's supposed to stop the machine if you fall. Not sure how well they work They're a PIB & easy to circumvent.
